MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home. Patients trust and rely on your judgment. And so do we! Someone with your compassion, clinical insight, and decision-making skills is ideal to oversee the recovery process of patients at home. Reporting to the Coordinator of Care Nurse, you will see patients in their homes for revisits based on clinical need. You will implement the plan of care and evaluate the treatment. You will document visits electronically and complete documentation within 24-48 hours. Qualifications: NY State Licensed Practical Nurse (LPN) license required. Minimum of one year of Med/Surg experience in a hospital or long-term care facility required. Driver's license and car required. Bilingual in Spanish, Mandarin, or Cantonese preferred.
