Location: Brooklyn, NY Base pay range $65,000.00/yr - $74,000.00/yr $10,000 Sign-On Bonus or Student Loan Assistance? Responsibilities Reporting to the Coordinator of Care, you will be responsible for seeing patients in their home for revisits based on a skilled need, implementing the plan of care and evaluating the treatment. You will document the visit electronically and complete documentation within 24-48 hours.
