The Opportunity:
Student Health Centers (SHC) is a unit within Integrated Clinical Services (ICS). The mission of the SHC program is to promote the whole community, whole school, and whole child health, wellness and readiness to learn. The Program operates 9 centers in coordination with five school districts: Centennial, David Douglas, Parkrose, Reynolds and Portland Public Schools, and the School Based Mental Health Program of the Multnomah County Behavioral Health Division. This position leads a State funded telemedicine pilot project with Multnomah Education Services District (MESD) to increase access to SHC services at schools that do not host SHCs. The Community Health Nurse (CHN) will use their clinical expertise to assist with planning, implementation and monitoring of this project to assure specific grant related activities and deliverables are met while meeting clinical standards. In collaboration with SHC leadership, this position is responsible for coordinating the project team and working with MESD. This position also has a clinical practice in SHC.
As the Community Health Nurse duties will include:
Leadership and Project Management
Lead multi-stakeholder telemedicine pilot project team
Develop, collaborate with stakeholders (MESD, school district, and State) and monitor adherence to project plan, schedule, budget, and medical/health deliverables
Document and report operational progress in partnership with Student Health Center leadership team for state and funding reporting requirements
Evaluation and Screening, Care Management
Assess client needs and telephone response for urgent and non-urgent medical information
Respond to emergency events in the health center and in the school (if 911 called first)
Administer medical treatments and medications
Direct and assign the care of individuals and groups of clients to members of the team based on scope of practice
Quality Assurance/Improvement
Conduct analysis on best practices and trends, and formulate recommendations
Complete assigned Quality Control/Assurance activities
Promote Student Health Center program

Minimum Qualifications/Transferable Skills*:
Education/Training:
Possession of, or ability to obtain, an appropriate and valid license to practice as a Registered Nurse in the State of Oregon
One of the following:
A Bachelor’s degree in nursing from an accredited nursing program; OR
An Associate’s degree in nursing from an accredited nursing program AND two years of registered nurse level experience is required.
BLS/CPR Certification required
Must pass a Fingerprint or background check
